A 24-year-old woman presented with a several year history of evolving 'lumps' especially on the central chest. The lady had mild facial acne and was otherwise well.
A 67 year old lady presented with an 18 month history of intensely itchy skin affecting the right outer elbow. There were no other symptoms. The skin had not responded to emollients and a range of topical steroids.
The itching and scratching were so severe that shallow ulcers had developed. There were no lesions elsewhere.
What is the diagnosis?
The diagnosis is brachioradial pruritus. There were no skin changes elsewhere, or a background of erythema to suggest an alternative diagnosis such as dermatitis herpetiformis.
Brachioradial pruritus and notalgia paraesthetica are two well-defined conditions causing localised areas of itching / burning. Brachioradial pruritus refers to an area around the elbow and extensor surface of the arm. Notalgia paraesthetica refers to the mid-scapular area. The aetiology is possibly that of a sensory neuropathy. Some cases of brachioradial pruritus are thought to be secondary to cervical pathology.

A 15 year old lady was referred for a diagnostic opinion for skin changes presenting in birth, affecting the right upper back, shoulder, antecubital fossa and dorsal wrist. The patient was asymptomatic and otherwise well.
